Simple Customer License page - Frontend

Looking for Frontend designer/developer, for a very basic Customer license page.

In attached PDF is a quick draft of what I need, there is Login page and then Main page, all the rest of the Forms should be pop-up forms, not new pages.

Looking for modern look and feel of the Main page.

Main page consists of 2 areas:
1. Left side with all the customer info, Products list is fixed to 4, and each selection shows lists of Licenses in the main view. If there are licenses for the Product, it shows number of licenses in brackets
2. Main view lists all available licenses, and when a License is selected it shows the list of Activations for selected license.

There are 5 buttons (if design calls for different element, please make suggestion) that open pop-up forms + 1x Refresh button that refreshes current License Activations list.

The backend has 3 main tables:
Customer table (CustomerID, Name, Country, MainContact, MainContactEmail, ISV details...)
Products (ProductID, Name) - I have fixed 4 products
Licenses table (CustomerID, ProductID, LicenseID, LicenseNo, Activated, MaintenanceExpireDate, Users)
Activations table (LicenseID, ActivationID, DateActivated, Userinfo...)

Backend is developed with PHP, I assume Frontend will be Javascript.

I prefer not to need to deploy some huge framework.
