Modern Form-Centric Landing Page

I have hosting of Namecheap EasyWP and my domain is live.
I need a concise, modern landing page that mirrors sastaticket.pk. The core of the page is a single form collecting From, To, Date, Type, and Weight, then posting that data to my existing portal endpoint (I’ll share the URL and field names).
Design & branding
• Overall look: clean, modern and fully responsive.
• I’ll supply our branding guidelines—fonts, colors, and logo—so the visuals stay on-brand.
Page structure
• Slim header with logo and Login / Sign-Up links.
• Hero section containing the form front and center.
• Light footer with basic copyright and quick links.
Form details
The five fields above are the minimum. I’m open to discussing one or two extra fields if you see clear UX value; otherwise, keep it lean. After submission the user should land on a simple confirmation screen or toast message—whichever you think feels smoother.
