Jotform Custom Sign-up Form Creation

Hello,

I need a Jotform sign up form created for our mentorship interviews. The form should allow applicants to select a specific interview date and time from available time slots for next week. I would like it to be clean, professional, and easy to use on both phones and computers.

Please include:

Multiple interview days
Specific time slots applicants can choose from
Name, email, and phone number fields
Automatic confirmation after submission
A way to prevent double booking of time slots

Once the form is completed, I will also need the form transferred over to me with full access and ownership.
