Build Automated Scoring Form + Email Funnel (Golf Coaching Business)

Job Description:
I am looking for an experienced developer or no-code specialist to build a fully automated assessment form for my online golf coaching business.

The goal is to create a simple, user-friendly form that evaluates a golfer’s skill level and automatically delivers personalized results and recommended coaching programs.

Project Requirements:

Assessment Form Creation
Create an online form (Typeform, 123FormBuilder, or Jotform)
Each answer must have an assigned point value
The form should automatically calculate a total score upon completion
Score-Based Logic
Based on the final score, categorize users into different skill levels (e.g., Beginner, Intermediate, Advanced)
Each category should trigger a different outcome
Automated Email System
Automatically send results to the user after submission
Email should include:
Their score
A brief explanation of what the score means
Recommended coaching programs/products based on their score
Bonus: Clean, well-designed email template that matches a premium brand
Admin Notifications (Optional but Preferred)
Send a copy of results to me (or store in Google Sheets/CRM)
Integration (Preferred)
Integration with email platforms like Mailchimp, ConvertKit, or similar
Ability to easily edit scoring, recommendations, and email content later

Ideal Candidate:

Experience with form builders and automation tools
Strong understanding of conditional logic and workflows
Experience with email marketing integrations
Bonus if you’ve built quiz funnels or lead generation funnels before
