Basic Excel Data Web App

I have a small Excel workbook that I currently use to keep track of records. I want to convert it into a lightweight web application so I can manage the same data online and stop emailing spreadsheets back and forth.
Scope
• Build a simple web app focused entirely on data entry and management.
• Provide basic authentication - can define who can edit what.
• Import the existing Excel file to seed the database (whichever is simpler and easier).
• List-view table that lets me search, add, edit, and delete rows.
• Option to export the current dataset back to Excel or CSV.
• Clean, no-frills UI; Bootstrap or similar is perfect.
