Microsoft Dynamics CRM Cloud Click to Call

We are using the Microsoft Dynamics CRM cloud and need a click to call solution and information popup/information loading automatically based on the calling number.
For Click to call we have a very simple API from our Cloud PBX Provider, where every user can initiate a call by entering the number or using tel : Link from any website.
Soon as the incoming call comes, need to fetch the data from IP- Telephone via web API ( SNOM D785 ) and
Search in the Microsoft Dynamics CRM Cloud and show the result, if the person is existing in DataBase or not.
