Visual Basic Print Label

I have a software which is written in visual basic .net (windows app) and I need to print a label. The label is 6" x 4". I need 2 buttons, send to printer and save as pdf. Label fields shown in template below, need to align a bit better. Can make a standard single window app in vb where we can fill fields and press print (send to default printer) or save pdf
