Reverse engineer Visual Basic 6 password generator

We have lost the VB6 code for our password generator which is required to create new databases for our legacy VB6 software system. The generator has 4 fields (Name, Address 1, Address 2, Suburb), a GENERATE button and a field for the resultant password. The password which is 4 or 5 alpha characters, is then entered into our software, and after verification, enables the creation of a new Access database.
The PASSWORD_CODE.TXT file contains the code in our software which verifies the password.
An extra alpha or numeric character is required to make the password 5 or 6 digits.
Can you reverse engineer the generator and extract the code enabling us to rebuild this vital component?
