Excel Macro -- 2

I have a spreadsheet of data that I would like a macro to add the data to rows on another spreadsheet. I want the ability to continually add rows. I have attached completed example. See tabs at botttom of spreadsheet labeled FROM (original data) & To (completed data). Thank you!!!
