Double-check and fix VB script in existing Word document

Hi, everybody.
Macro-enabled Word document that opens a UserForm automatically. When the user submits the competed the 6 fields in the UserForm, it creates a formatted document ready to save or print.
Additional Information
I need the existing macro-enabled Word document UserForm fixed.
-- check code for the Submit button so that the input to the UserForm fills in the document.
-- create code for the Cancel button so that it closes the UserForm and returns to the document.
-- add "info" icons to the UserForm with MouseOver Tooltips (see attached document)
-- Fix issue UserForm does not open on new document.
If you take a stab at the code that would save the document to a path with a filename from the Author field, as I put in one of the documents. If you can accomplish this, that would be great.
That's optional, though. The basic document with the UserForm is fine, if you want to keep it simple.
