Convert a VBA Macro to a COM add-in

I have created a VBA Macro in MS Outlook which, using a UserForm, gives users a choice of which folder in windows explorer they want to file emails and attachments down into. I now want to distribute this Macro as a COM add-in. I have tried creating this in VS Studio but, due to time constraints, I need someone to sort this sooner. I can provide the VBA code and User Form. I just need the functionality duplicated.
