Segmented Text Edits in MP4 -- 2

I have an MP4 video that already contains on-screen titles and captions, and I need each occurrence of that text replaced with new wording. The required changes are not uniform—each segment of the video calls for its own specific replacement—so attention to timing and consistency in style, font, and animation is important.
Please work directly on the source file, preserve its resolution and audio, and deliver a clean, render-ready MP4. If you prefer to swap only the text layers through Adobe Premiere, After Effects, or similar software, that’s fine as long as the final export is seamless and artifact-free.
I’ll provide:
• The original MP4
• A time-coded list showing the exact text that appears and what it should be changed to
You’ll return:
• A revised MP4 with every requested text edit in place
• (Optional, but appreciated) the project file so I can make minor tweaks later if needed
