Nursing Assessment Demo Video

This assignment is a video head-to-toe physical assessment on a client, 18-years-old or older created with YouTube. Your examinee must be competent to give consent for the physical. They must be able to follow directions. Please make sure your examinee understands that this is a head-to-toe examination. Your examinee may wear shorts, a tee shirt/tank top and a gown). You will not be doing a breast or genital exam. You will provide privacy but the instructor must see everything the examiner sees. Students will have 50 minutes to complete the physical assessment. Anything recorded past that time will be given a zero. Recording time during camera adjustment of the viewing area and looking at a two-sided sheet of notes are not counted in the 50 minutes. Students are to conduct the physical in the order on the rubric, but if something is forgotten, they may go back if time permits. Students will be allowed to look at the notes twice during the exam and must remain in the frame of the video while looking at the paper. Students will slowly scan the room 360°, with the camera, at the beginning of the recording. (There will be a 5-point deduction if the room is not scanned.) The instructor will grade the technique, assessment interpretation, systematic approach, professionalism, pronunciation, and interaction with the patient. Students and clients may use aliases but the student should report this to the instructor when the assignment is submitted. You will have 50 minutes to perform the assessment. You may have one sheet of paper (may use both sides) no larger than 8.5 x 11 inches, for reference. You may write an outline of your assessment or other reminders to assist you with the exam. You will be allowed to look at your paper twice, at any time during the assessment.
You will not do a write-up to document your assessment findings; rather, you will “talk your way through the exam”, giving the evaluator your findings as you progress through the assessment. Make sure you give specific findings, do not use the terms WNL, normal, fine, great, or no abnormalities. Describe what you observe, feel, and hear. All techniques and assessments MUST be in view of the camera. You will receive a zero for anything the instructor cannot see you do. Therefore, you must move your patient, camera or both. It is suggested you have someone else operating the camera. Sound must be audible during the exam.
Students will be evaluated on completeness of the exam as well as technique, assessment interpretation, systematic approach, organization, appropriate draping, pronunciation, interaction with the patient, and consideration for client comfort and privacy. The student should exemplify a professional nurse in attire and behavior. Required Equipment

2 Spices (to test for senses of smell)-do not use pepper of any kind. (Coffee or peppermint oil can also be used – do not use any spice “patient/client” is allergic to.)
Tongue depressors
Cotton tip applicators (used to test sharp/dull)
Stethoscope (with bell and diaphragm)
Cup of water
Reflex hammer
Tuning fork
Coin, paper clip, safety pin (to test for stereognosis)
