Metal Nameplate Engraving Design

I need a designer to create a memorial plaque design for steel laser engraving/cutting.
Project Details:

I have 17 nameplates (Each name nameplates 11 Name including) in an Excel sheet (Rank, Name, Unit, Date of Death)
Each entry needs to be in a rectangular bordered box
Military-style bold font, all uppercase, centered text
Dark background with light/white text (for laser engraving preview)
I will provide a reference image showing the exact style I need

Deliverables:

Vector file format: SVG, DXF, or AI (laser-cutter ready)
High-resolution PDF or PNG preview
Editable source file

Requirements:

Experience with laser cutting/engraving file preparation
Must match the reference image style 100%
Clean vector lines suitable for CNC/laser machines
