Fix VBScript Popup Modal Error

I have a small form that relies on a VBScript + JavaScript snippet to show a simple popup with two buttons—OK and Cancel—whenever the user clicks Submit. It works locally, but once the site is published the browser throws a “modal dialog” error and the window never appears.

The task is straightforward: identify the root cause of that error and deliver a reliable, cross-browser solution that still shows the same two-button dialog and carries out the existing logic tied to each button. I need the fix implemented directly in the current script; a full rewrite isn’t necessary.
