UX/UI - Designer

I need to create a sign up page that will have multiple choices for the person registering. It will be simple but a very important page as we will be using it at a convention to get people to sign up for different groups.

It will need to be very mobile responsive and very easy to use. I am looking for someone that has experience in both UX/UI and be able to design it
