Wellness Survey Structure Audit

I have drafted an online survey for a new line of mental-wellness products and I need a fresh set of expert eyes on its structure. The survey’s main purpose is to gather user feedback from potential users before we launch, so every question needs to be clear, bias-free, and logically sequenced.

Here is what I’m looking for:

• A detailed critique of the current question flow, wording, and answer scales.
• Suggestions to shorten or consolidate questions where possible while still capturing actionable insights.
• Identification of any leading or double-barreled questions that could skew results.
• Recommendations on skip logic, branching, and mandatory/optional settings to keep the experience smooth for potential participants.
• A revised version of the survey delivered as an annotated document (or directly within Google Forms/Typeform if that’s easier), showing tracked changes and comments so I can see exactly what was adjusted and why.
