Online Admission Form to populate our Student Information System
Budget: $3,000 – $5,000 CAD
What we're looking for:
An experienced freelancer to build and design an independent (sub-domain off our website) web application form on a LAMP(preferable but not absolutely mandatory) stack. Once submitted the form will populate our Student Information System.
The FORM is an application form for prospective students to apply to our institution. The form itself request approx. 40-45 fields of input, with some dynamic logic applied, IE if they say they have attended a School before, we then also need to know what school, program, and additional subsequent information of that selection. – A field schema will be provided for form fields, and submission file requirements.
A login routine will be required as the form will need to have the ability to be saved and allow the user to return at later time.
When the user submits the application, we require 4 actions:
1. A flat file is created for import into our SIS
2. A formatted write-protected PDF is created.
3. The above-mentioned PDF is sent with a confirmation email to the user/applicant
4. The above-mentioned PDF is sent to a storage location within our enterprise in a secure format.
Support to validate, troubleshoot, and ensure the data is successfully imported into our Student Information System.
Design considerations are minimal, we require a professional look, but do not need a fulsome design centered around the form, some basic(provided) brand guidelines will be provided. – IE the form is the only purpose of the website/applicaiton
We need the following deliverables:
1. The web form with logic
2. Database functionality
3. File creation and SIS importing
This is our second attempt to outsource this project so preference will be given to freelancers that can complete with a short turnaround
To complete all the deliverables, the right developer will have experience in the following:
• Experience designing forms – Wireframe would be nice but not necessary
• HTML/CSS/JavaSCRIPT/PHP/MySQL
• PDF and Flat file creation experience
