Build me a mobile app -- 2

I run a child protection organisation in South Africa. I want to give parents training on 3 pieces of legislation relating to family safety. The app must
1. Register the user
2. Link to 5 training videos on youtube (and monitor when watched)
3. Link to 3 statutes on a dept. of justice website (and monitor when read)
4. Give a test on each video (5 x 50 multiple choice questions) before and after watching it
5. Include a repeatable survey/questionnaire (50 questions)
6. Export a certificate of competence using the scores from the 2 tests (if they complete the questionnaire)
7. Export a certificate of the questionnaire results
8. Collect user data, useage stats, scores and replies centrally
9. Google ads
