design and implementation of PostgreSQL database from an existing schema

I am looking for someone who can help me with an existing database project in the medical field.
All that is currently available is a drawn diagram of a possible physical model of the database in question and a document with some rough specification information.
The aim is to reverse engineer and design a conceptual, logical and then physical ER model of the database in question, resolving all the conceptual and design errors that exist by following best practices and optimising everything.
The required output is the ER schemas, the physical model and the SQL queries to be executed in PostgreSQL to implement the designed database.
