Digital Registration Form Design

I need a digital registration form turning form a pdf we currently have to a digital form that will have fillable fields that we can fill in over the phone with a customer to document customer requirements and needs and then email to customers for e-signature.

Requirements:
- Take and existing form that is in pdf format and make into a digitally fillable version
- it must be easy to use for someone that isn't super tech savy
- E-signature integration is a must
- User-friendly and professional design

Ideal Skills and Experience:
- Experience in form design
- Familiarity with e-signature tools
- Strong UI/UX design skills
