​Specific Instructions for the Editor:
​1. General Layout & Typography
​Font: Use Arial or Helvetica (11pt or 12pt) for the body text.
​Margins: Standard 1-inch margins on all sides.
​Cleaning: Remove all technical artifacts including bracketed source tags (e.g., ``), dashed page markers (e.g., --- PAGE 1 ---), and bracketed instructions (e.g., [PAGE 2: CONTINUED]).
​2. Formatting Page 1: The Nexus Letter Header
​Clinic Identity: Center the name KEVIN KO, M.D. in bold (16pt) at the top.
