I need a designer to create a 4‑panel Foldable Reference Card (Blank Fields) and fully editable by customer

I’d like a compact, foldable reference card designed — 4 equal panels in a row (accordion fold to a single-panel size), double-sided, with blank fields only (placeholders/labels, no prefilled data- fully editable on customers end).
Please follow the attached example’s proportions and dense grid-style layout.
Approx Quick specs:
the card (fully opened as shown) is about 7 inches wide by about 13 inches tall (≈20 cm × 30 cm). Folded into quarters
- Layout: dense grid with thin rules; rectangular fields (single-line entries, multi-line notes, two-column key/value rows, small checkboxes). All fields empty; include slim colored section headers.
- Typography & color: condensed sans-serif (e.g., Helvetica/Roboto Condensed). Headings ~7–8 pt, body/labels ~6–7 pt (adjust for legibility). Black text on white with one accent color (suggest navy/royal blue). CMYK-safe
