Transcribe Forms & Surveys to Word

I have a batch of physical forms and survey sheets that need to be keyed into a clean, well-formatted Word document. Everything is on paper, so the task is strictly manual data entry—no OCR or automated tools here.

Each form contains a mix of tick-box answers and short written responses. I’ll supply high-resolution scans; you’ll type the content exactly as shown, preserving spelling (even if it looks odd), and lay it out so every field from the original is easy to trace in the new file. I expect consistent font usage, logical headings, and page breaks that follow the structure of the original documents. Accuracy is far more important than speed, so double-checking before you hand the file back is essential.

Deliverables
• A single Word (.docx) file containing all entries, formatted to mirror the originals.
• A brief log noting any illegible words or ambiguous marks you encounter.
