Client Intake Form Creation

I need a comprehensive intake form for my cleaning business to collect essential data from clients. The form should be user-friendly and efficiently gather the following information:

- Contact details
- Service address
- Detailed cleaning checklist
- Company Logo
- Be editable for business changes

I have attached a template that I would essentially like updated with my company information and logo and tweaked to include the contact details of the client (service address, square footage, numbers of beds/baths.
