Scope of Work
Build a polished Introduction / Welcome Page using my content and branding.
Create a dynamic questionnaire with multiple respondent personas using branching logic (Health & Wellness Enthusiasts, Coaches, Dietitians, Retailers, Restaurants, Gym Owners, Healthcare Professionals, Corporate Wellness, Students, etc.).
Implement conditional logic so respondents only see questions relevant to them.
Build a Registration Page to collect contact information and interest in future programs (waitlist, founding members, alpha/beta testing, partnerships, etc.).
Create a professional Thank You page.
Ensure the entire experience is optimized for mobile devices, as users will access it by scanning a QR code on a business card.
