I need a single, well-structured “Contact Us” page that represents my business online. The stack is fixed—plain HTML5 templated with Bootstrap, front-end validation in JavaScript, and a small PHP script that writes every submission into a MySQL table and fires an email notification.
Please make the layout fully responsive, keep the markup clean, and respect Bootstrap’s utility classes so I can expand the site later (Home or About pages could follow).
