tayloring a shirt

I am looking for someone to tailor two polo shirts for me, for personal use. No alterations are required. I am hoping to find someone that has the skills and experience necessary to complete this project within an agreed-upon timeframe and budget. This project does not relate to an existing business or a new enterprise.

The Shirt has to look like the picture with sticks. One in Black and one in white.

The mesures I will provide later.
