Translate a patent Specifications from English to Polish

I am looking for a professional translator to translate a technology-specific patent from English to Polish. The specifications are about 8K. In addition, a 1 page letter needed to accompany the translation with the patent information such as Grant date, Patent title etc.

– the name and address of the representative,
– the IPC,
– a signed declaration stating that the translation corresponds to the original text,
