Hebrew Lease Keywords Translation

I need someone who is proficient in both Hebrew and English to translate some keywords which is handwritten from a lease document. It is just about 30 words and for personal use, so it doesn't need to be certified (attached the file - need translation only for the handwritten part). Also, I need ongoing support.
