French Translation of Labor Survey and Consent Form

I'm in need of a native French speaker or trained translator who can help me translate a labor-focused survey and consent form. The survey comprises of 19 questions and the consent form is 2 pages long.
Requirements:
- Translate a 19-question survey and a 2-page consent form (the consent form has my personal information so I did not include it here)
- Attention to detail is crucial to maintain the original meaning and context
