Word Report Template

I need a word report template created. I’ve been using the attached as a template but it is riddled with formatting issues and to be quite frank is causing me a great deal of stress! I need a robust template created along similar lines to the attached perhaps given a bit of a more contemporary style/approach that I can rely on when writing reports.

Is this something you can assist with. If so please can you let me know costs and timescales?
