Template for on site quotations and job information with tabs, drop down menu's, checklists, yes/no selections, attach photos

We are after a template that can be filled out on job sites using a mobile phone or tablet. It will allow us to attach all relevant information regarding the jobs for the purpose of both the workers and to be easily forwarded to clients in the form of a quote, some of the information will be

*Each TYPE OF JOB will have about another 10 options e.g multiple choice/drop downs/yes or no selection boxes or text boxes relevant to that scope of works.

I am not sure what file type, program to use but it has to be able to be sent to clients via email and contain photos. There are subscription services that offer a similar product but we do not want to pay ongoing costs. Any recommendations and information is welcomed.
