Create progress reports of students

I need a freelancer who can create progress reports for the students. We have data of students which contains chapterwise progress. We have the following data. This is going to be used for desktop application using Microsoft Windows OS
1) Total number of questions available in the chapter.
2) Number of questions attempted
3) Wrongly and rightly answered questions ( Numbers)
4) Date and time of activity and results.
5)standard/subject/chapter name

We need a freelancer to create templates and show results to parents and students in an easy manner.

Freelancer should show psudo templated.
