Lead Generation of Many Verticals

PLEASE READ COMPLETELY!!!
do not ask my budget read what i need give me your price based on that

i am also open to anybody who is selling INBOUND calls (consumer dialed) for ANY vertical
tax debt - credit card debt - health insurance - obamacare/aca - water damage - etc

I am also open to any call center who are generating TRANSFERS

so i am open to buying inbound calls - transfer calls - and consumer data
I am NOT looking for anyone to set up ads for me

I am only interested in CONSUMER leads (possible customers, people who are interested in buying the service) this is NOT BCB leads

I need someone who can generate leads in the following verticals
Health Insurance (Under 65)
Medicare
Obamacare (low cost health insurance)

this will be for an outbound call center
must include following headers
Name - Email - Birth date - Phone Number
I prefer jornaya/trusted form data but I am open
headers can also include (income)

please use the phrase "LEAD DATA" so i know you read and understand what i need

tell me the minimum amount of leads you will sell and at what price
also connectivity (answer ratio matters) can't have a much of answering machines or disconnected numbers
this will be a long term gig if the leads are good i also want to know if you sell real time leads or aged leads and i need to know how aged NO re-sold leads that have been sold over and over again

in your bid please tell me the min # of leads - price for the leads - included headers - if its jornaya/trusted form - type of data (medicare/health/obamacare)
