Create an interactive Adobe PDF for tablet use to document vehicle damage. The form allows users to add photos dynamically; each inserted photo/taken photo links to a damage-description field offering a selectable list of damage types. Selecting a damage type auto-populates its associated price.

Additional photos can be added via (+) and each selection updates a running total displayed at the bottom, calculating subtotal, VAT, and grand total.

Include two top fields for Vehicle Registration and Date, and two signature boxes for Customer and Operative.

Description line populated next/under photo once uploaded that automatically selects corresponding price
