Self-Assessment Goal Alignment

I have already drafted most of my annual self-assessment form, but I am struggling to express how each answer ties back to my agreed professional development goals. I am looking for someone who can walk through the document with me, challenge my wording where needed, and make sure every statement demonstrates clear alignment with those goals.
Here is what I need over a short engagement:
• A quick review of the existing form (Word or PDF).
• Constructive, targeted edits that connect each section to its corresponding professional goal.
• Suggestions on measurable language so my achievements read as outcomes, not just activities.
• A brief follow-up call or written summary confirming that all responses now map cleanly to the stated objectives.
Experience with performance evaluation language, competency frameworks, or HR-style development plans would be ideal. Please let me know how soon you could start and how you like to collaborate on edits (Google Docs, track changes in Word, etc.). I would like to finalise the form within the next few days.
