Professional musical feature request letter

Im terrible at writing. I need a professional letter that i can send to musician managers to request for a feature from their artists. I need it to say who its on behalf of, asks for pricing for a feature, and be a prototype that i can easily change to send to different managers. Preferably in the style of an email. This will be to notable public figures.
