Engaging Customer Satisfaction Survey

I'm looking for an expert to format a survey in SurveyMonkey focused on evaluating customer satisfaction. I have a completed survey of approximately 15 questions.
- I am looking for professional formatting, limited scrolling between questions, and a nice looking header with our logo.
- The individual in my office who built it is new to SurveyMonkey. She is doing her best but it doesn't look quite as professional as I would like it to.
- I would also like advice on how to improve our question phrasing to illicit better responses.
