Create post-tour surveys

We wish to replace our paper based survey system with one that can be emailed to clients upon their return from their tour with us. There are up to 22 clients per tour and we would like to be able to consolidate the results for each tour and then compare those consolidated results on a tour by tour basis. However each tour survey will need to be amended to record the name of the guide & tour manager. Names of hotels will need to be added and this can vary from only 1 to up to 8 depending on the duration. See attached blank survey for an idea of what we are trying to achieve.
