Capture survey response

I want to run a survey and capture response for a set of 4 multiple choice questions. The exact question will be shared with shortlisted freelancer.

If you want to bid for this project, you must tell me:
- How will you target audience for the survey
- Which demography, region will you target
- Which tool you will use
- How many responses can you capture
- Cost & time to run the survey
- Examples of surveys run in the past and its result

Scope of work includes designing the survey to make it attractive, creating the right content, setting the tone, running the survey & modifying for optimum response, capturing response in exportable excelsheet, mapping demography / region of users, etc.
