Medical Survey Descriptive Statistics

I have a set of medical survey results exported to Excel and need a clear, well-structured statistical summary. The focus is strictly on descriptive statistics—specifically:
• Mean and median values for each key variable
• Distribution and frequency tables (or pivot charts) that let me spot patterns at a glance
Nothing beyond those two areas is necessary; trend or correlation work can remain out of scope for this task.
You will receive the raw survey file in .xlsx format. Please clean any obvious data-entry errors, document every transformation in a separate “Data Prep” sheet, and then build the summary on its own tab. Visual aids such as histograms or bar charts are welcome if they make the frequency insights easier to interpret.
Final deliverable: the updated Excel workbook, fully unlocked, with formulas intact and an optional brief note explaining your approach so I can replicate it later.
