Excel Patient Records Statistics

I have a spreadsheet of patient records that needs a clean, straightforward descriptive analysis in Excel. Specifically, I want to understand the basic shape of the data by calculating mean and median for key numeric fields, and creating clear frequency distributions for categorical columns.

You’ll receive the raw workbook exactly as exported from our EMR system. Your task is to tidy any obvious entry issues (dates as text, blank rows, stray spaces), set up the formulas or pivot tables, and deliver:
• A summary sheet showing the mean and median for the selected numeric variables.
• Frequency tables (or pivot charts) that display how often each category appears.

Keep everything native to Excel—no external add-ins—so the team can maintain the file internally after hand-off. Please document any data cleaning steps directly in the workbook so we can trace changes later. A short walkthrough video or notes on how to refresh the analysis when new records are added would be a welcome bonus.
