add address to a mailing sticker template

I'm seeking an experienced freelancer to add an address to a custom sized mailing sticker template in the traditional (street, city, state, ZIP) format. No additional information is needed on the sticker. X 45 addresses given via a Word Doc and to be added to a Avery Sticker template provided.
