yellow boxes, square parenthesis

I am attaching my document that I created based on the 10-15-2019 document. It's called Tangipahoa Parish School System Application Form. I have not inserted the blocks, squares and other pieces. Please let me know if you have experience with creating fill-in forms in Word and think this can be done.
