I need a skilled freelancer to create a new booking form on my Squarespace website that integrates directly with MechanicDesk using their booking widget. The form should efficiently collect customer information and vehicle details, and synchronize appointments with MechanicDesk’s real-time availability.

Key Tasks:
- Create a new booking form in Squarespace to capture:
- Customer name, email, and phone number
- Vehicle details (make and model)
- Service type
- Preferred date and time
- Integrate the form with MechanicDesk using the provided booking widget embed code.
- Ensure that appointments appear in MechanicDesk and that the date/time slots reflect real-time availability.

Skills Needed:
- Experience with creating and customizing Squarespace forms
- Ability to integrate third-party widgets into Squarespace
- Familiarity with MechanicDesk’s booking widget and its functionality
