Webpage that will Graph data from SQL

The website will used internally on company network, not public.
What we are looking for is a webpage that will retrieve a couple of data points and show a graph for them. The data will be in Microsoft SQL.
Example: Item is selected. Item's current data point values will shown, and the a graph will display when a data point is selected showing the past values of the data point.
