Setup SQL DB

I want someone with experience in SQL DB (lite/express initially) creation and population from web forms. The web forms are on my soon to be live website. Once completed the data can be sent via email to a folder that will be polled regularly, (or by web service link) to the DB. I am looking for someone that is not just looking for a one-off gig. I will have a massive amount of work into the future - so looking for someone that i can build a long term relationship with. Future work will include reporting, email/web service integration and logic, as well as a huge amont of other things. This initial small piece is just to feel out how you work, and your ability, timing, trust etc
