Revamp old database to new -- 4

We would like to revamp our old patient database that runs on Microsoft access to a new smart database that would be able to merge all our databases together, create specific reports, a way that our admin team can add fields if we need to. We will also need the system to be efficient and user friendly. There are more specifications that needs to be discussed and can be done on the chat. Its like a mini clinic management system where the practitioner would be able to store scanned medical reports as well. Something similar to what we are looking at is Cliniko.com
