PHP Referee System Development

I need a PHP-based referee system built with the following components:

Main Screen:
- Black background
- Green dots indicate answers from Left/Right/Center until all have answered
- Answers displayed as white/red dots
- Countdown timer at bottom, controlled by gray button on center screen
- Timer resets after answers revealed

Phone Redirect:
- Redirects referees to assign Left/Right/Center roles

Left/Right/Center Screens:
- Black background
- Red/white buttons submit answers
- Gray button on center for timer control

Requirements:
- 5 HTML/PHP files
- SQL file for database

Experience with PHP, SQL, and front-end development is ideal.
